Welcome to the thirteenth Carer's Voice Questionnaire. Thank you for helping us to learn more about the services which affect you as a carer. This questionnaire covers the following topics:

Section 1. Experiences with crisis support - this section covers the most recent situation when you have unexpectedly needed additional help to look after the person you care for. For example if you suddenly became ill or had to deal with another family member, or if the person you care for suddenly became more ill.

Section 2. Experiences with Wiltshire Council's reablement team or the PCT's neighbourhood team - this section covers the most recent situation when you had contact with either Wiltshire Council's reablement team or the PCT's neighbourhood team. The reablement team provide short term support to help individuals to have the skills to live as independently as possible, neighbourhood teams provide health care in your home.
