Reduce the numbers of people aged over 75 years and over who are admitted to hospital as a result of a fall by 150 (839).
The figure is calculated using cds inpatient data, as submitted to HES. This data must be validated by HES before being used. Interim data can be used and calculations confirmed approx 4 months after years end.
Currently we are analysing data for year ending 31st March 2007.
2005/06 figure - 1164. Progress against the target is very limited.
The Pathways for Change process which plans total change in models of care mean that original structure for meeting targets needs review, however the target achievement is one that meets the vision of the PCT generally so remains a priority area for the PCT.
Further changes to the management structure and financial recovery measures have delayed progress, and although this area of work remains one of the organisational priorities, other priorities have superseded it. The intention was to focus action on this in the New Year (Jan 2007) with a new management and professional structure, although this action still remains to be undertaken. Figures are now available for 05/06 (1164) and by mid-May will be available for 2006/07.
