The Home Care Re-Ablement Service is a domiciliary care based service that aims to maximise independence and promote choice and well-being by helping people to regain the skills and confidence to undertake tasks themselves, rather than relying on a domiciliary care worker to complete the tasks on their behalf.
The Service provide a home care based intensive period of close working (normally six weeks but in exceptional circumstances, and based on the individual needs of a Service User, it could be a maximum of ten weeks) with people in their own homes.
Typical tasks could include, assisting an individual to use a microwave enabling them to prepare a meal for themselves or helping them to relearn the skills required to make their own bed and / or to do the laundry.
The Service will operate between 7am and 10pm, 365 days per year. It will be staffed by highly trained personnel who are dedicated to their job and undertake the activities whilst respecting the needs, wishes and dignity of those they assist.
Individuals must be referred to the Home Care Re-Ablement Service by a member of the Council’s care management team, following a comprehensive assessment of their needs.
Assessments can be arranged by either contacting the Social Care Help Desk, an individual’s designated care manager in the Community Teams, or through the hospital discharge process.
The Home Care Re-Ablement Service will be accessible to all adults who require the service. This will include older people, and those Service Users with a learning disability, mental health problem or physical impairment. Furthermore, the Service will be available to individuals who are able to fund their own care as well as those that receive assistance from the Local Authority.
The Service will recognise the cultural or religious preferences of the Service Users and attempt to be as sympathetic as possible to the provision of a Re-Ablement Service in line with these beliefs or customs.
Financial contributions towards the Home Care Re-Ablement Service are determined by a financial assessment that will be undertaken by a member of the Financial Assessments and Benefits team, prior to the commencement of care.
If financial contributions apply, an invoice will be sent to you every four weeks and can be paid by cheque, postal order or direct at any bank. A standing order service can also be arranged.
