About this service

The Adult Placement Service offers long-term care or support to people who are unable to live in a home of their own, but who would like to live alongside other people in a family-like setting.

The service also offers short-term placements: for example to provide a break for carers, or as a stepping stone for someone returning to the community after being in hospital.

We also provide a small number of “family-kinship” or “outreach” placements: these offer support and guidance to people who live semi-independently in the community.

The Adult Placement Service is registered with the Care Quality Commission (CQC)

How do I access the service?

The Adult Placement Service is used mostly by people with learning disabilities, head injuries and other physical disabilities, mobility impairment, users of mental health services and older people.

People in adult placements (known as “Adult Placement customers”) have a wide range of abilities and support needs. For example, some are fairly independent and only need guidance, reminders or explanation. Others need assistance with most day-to-day activities.

What can be offered without an assessment?

The Adult Placement Service is required to ensure that a person’s care or support needs are assessed before a placement is made.

Will I be charged for the service?

A financial assessment is made by Wiltshire Council to assess how much the person is able to contribute towards the cost of their placement.

How can we help you?

Once a suitable placement has been found, the Adult Placement Service provides support and advice to the Adult Placement Carer and monitors the quality of the service being provided.
