Introduction

The Community Connecting Team (CCT) works in partnership with people so that they can have the life they want in their community, one that both they and their community will value. This may be through finding voluntary work, socialising, joining clubs, going to their local college, or using their community facilities to do things that they enjoy, such as meeting friends.

The Community Connecting Team provides a time-limited service, with the aim of working with the individual to be able to do their chosen activities independently. If it becomes clear that the individual will need on-going support, the team can help them apply for a direct payment to purchase this.

How can you access this service?

  • You must be between the ages of 18 and 65
  • Have a learning disability
  • Live in the county of Wiltshire
  • Have a referral from a social worker or your day service.

Direct payments

Direct payments are money given to you by Wiltshire Council so that you can buy the support you need to live independently, rather than having support arranged by the council. You can use a direct payment to arrange the type of support you would prefer. For more information see our direct payments fact sheet.

What can you expect from the Community Connecting Team?

When you are initially referred to the CCT we will spend some time getting to know you and find out what type of activity you wish to pursue. We will support you to explore opportunities in your community, work with you to identify your existing skills, and help you to develop any new ones you need. We then plan with you how you are going to get to your chosen activities and will support with this you for the appropriate length of time.

We will review our work with you regularly. We are committed to providing you with a high level of service and expect, in return, that you will attend any meetings planned or inform us in advance if you are unable to attend.

Making a complaint

If you are unhappy with the service you are receiving you should first discuss the problem with your Community Resource Worker. If you tell them what is worrying you, they will try to find a solution. If the problem persists, please let us know.
