About this service
What is the Reablement Service?
The Reablement Service is intended to maximise independence and promote well-being by helping people to gain or regain the skills and confidence they need to live at home. The service is often needed following hospital discharge, bereavement or a crisis in the community. It provides a home-based intensive period of close working with people at home (normally six weeks but in exceptional circumstances it could be a maximum of 10 weeks).
Typical tasks could include, teaching a person to use a microwave enabling them to prepare a meal for themselves or helping them to relearn the skills required to wash, get dressed, make their own bed, do the laundry, use transport, or organise shopping.
The service operates between 7am and 10pm, 365 days per year. It is staffed by highly trained personnel who are dedicated to their job and undertake the activities whilst respecting the needs, wishes and dignity of their clients.
Who will be eligible for the Reablement Service?
People can access the Reablement Service through a hospital social worker or a community care manager. The Social Care Help Desk at County Hall can also advise how to make contact.
The Reablement Service will be accessible to all adults who require the service. This will include older people, and people with a learning disability, mental health problem or physical impairment. Furthermore, the service will be available to individuals who are able to fund their own care as well as those who receive assistance from the Local Authority.
The service will recognise people’s cultural and religious preferences and will work according to the Council’s anti-discriminatory policies.
Will financial charges apply to the Reablement Service?
There will be no charge to users of the Reablement Service. At the review in the home, towards the end of the reablement programme, people who need ongoing domiciliary care will be transferred to a long-term provider of services. For the long-term service there will be a financial contribution which will be assessed by a member of the Financial Assessments and Benefits Team. People who are totally self-funding can also request assistance in seeking a service provider that meets their needs.
