Shared care for adults

About this service

Wiltshire Council Adult Placement scheme provides care and/or support to Adult Placement Service Users in the homes of formally approved individuals, couples or families. Where necessary, or preferable to an individual Service User, the service may be provided in the Service User's own home, or elsewhere in the community, by an approved Adult Placement Carer.

A choice of Long-Term, Short Term, Respite, Outreach, Family Kinship and Day Placements will be offered to people aged 18 and over from the following Service User groups:

  • People with learning disabilities
  • People with mental health difficulties
  • People with physical disabilities
  • People who have been discharged from hospital
  • People with drug and alcohol dependency (or recovering from) and
  • Older people aged 65 and over.

Other people to whom we may agree, at its discretion, to offer the service (e.g. people with serious or terminal illnesses, who do not require nursing care from their Adult Placement Carer).
