Transition means change and during our lives we will all go through a number of changes.

Moving into adulthood is one such change and Transition Planning makes preparations for this.

All young disabled people who have a Statement of Educational Needs, in school year 9 (usually when the young person is 14 years old), will begin to be supported to plan for their transition to adult life. This support is called Transition Planning and it will continue until the young person leaves school and they are settled in whatever comes next and on into early adulthood if required. Young people on School Action Plus can also be offered support with Transition Planning.

A number of agencies can be involved in Transition Planning, with schools having lead responsibility for the process. However, it is young disabled people and their families who should be at the centre of any decisions that are made. Planning for transition should include education, employment, housing and support needs, community and leisure activities and health needs.

The purpose of this planning is to ensure continuity of care, and to make sure that the young person has access to all the information and opportunities that they are entitled to, or want, at a time that is meaningful to them.

In Wiltshire there is a Multi Agency Transition Protocol which describes which the roles of the various agencies involved in transition and how they should work togther to support the young person and their family at the centre of the transition planning process.

Moving On - is a quarterly newsletter aimed at both parents and professionals to keep them up to date with the changes that are being implemented to improve the way that transition is managed in Wiltshire.
