
The Health Improvement Partnership Board
The Health Improvement Partnership provides the main forum for the strategic overview and planning to improve the health and wellbeing of the population of Wiltshire.
Priorities for the Health Improvement Partnership are:
- Improve working across health and social care to provide more appropriate and local services, to help people recover, prevent hospital admissions and provide alternatives long term care
- Improve access to assessments and services for carers, and involve carers in developing services
- Support more people through direct payments/individual budgets to manage their own care
- Improve access to services for people with dementia
- Improve access to primary care services for people with learning difficulties
- Improve services for disabled children and young people
- Encourage people to make positive lifestyle changes
- Implement a universal Chlamydia screening programme for under 25s
- Work to understand deprived areas health needs and wants, implement a preventative scheme to influence the main causes of ill health and early death
- Working with children and young people to improve sexual health, support mental well being by early attachment, promote resilience, reduce substance misuse, tackle obesity and improve healthy eating and exercise
