Shared Lives Home from Hospital

Overview

Home from Hospital is a service that helps people, who are medically fit for discharge from hospital, to continue their recovery in a safe environment and regain the daily living skills and confidence they need to return to their own home.

Every day, people remain in hospital when they no longer need to be there. Home from Hospital helps people stay, and be cared for, in the home of a Shared Lives Carer for up to 12 weeks before either returning to their own home to live independently, or moving to somewhere that can provide the support they need.

Who needs care

Being a Shared Lives Carer is really flexible. You can support up to three people, working hours that suit you, and that fit around your family life.

Our Shared Lives Carers support people leaving hospital who aren't ready to return to their own home. This could be because their home is not suitable, they require day to day support to continue their recovery, or because there is no one at home to assist them. There are lots of reasons why some people are unable to return home straight away, including:

  • people who are waiting for the start of domiciliary care services
  • need more time to recover following medical conditions such as a stroke or needing support to learn how to manage a long term medical condition
  • people who have had a period of mental ill health which required a hospital admission
  • older people requiring more time to recover and regain their strength and
  • confidence following a hospital stay
  • waiting for their house to be made suitable

What carers do

Being a Shared Lives Carer is really flexible. You can support up to three people, working hours that suit you, and that fit around your family life.

Home from Hospital Shared Lives Carers:

  • help people to regain daily living skills and confidence
  • help people to be as independent as possible after a stay in hospital
  • help people to avoid readmission to hospital
  • prevent people from having to move into residential home until they need to

Shared Lives Carers do not need qualifications but should have a wide range of skills and life experience. Previous caring experience is not needed; we will train you.  

As a Shared Lives Carer, you would support people leaving hospital to regain daily living skills, fitness, and confidence to enable them to return to their own home. For example, depending on the person's needs, you might assist them to regain the following skills:

  • cooking
  • washing-up
  • cleaning
  • taking medicines
  • getting in and out of bed
  • dressing
  • washing
  • doing your shopping; or
  • getting out and about to see family and friends

Shared Lives carers come from diverse backgrounds. They may be single, or a couple, with or without children living at home. They can be any age over 18 years.

What carers receive

Being a Shared Lives Carer is really flexible. You can support up to three people, working hours that suit you, and that fit around your family life.

Carers receive support and training to gain care certificate - Oliver McGowan levels 1&2. Personalised, specific training is also available.

We run a Shared Lives forum every month where you can meet with other carers, and customers.

You will be self-employed, but we will be with you every step of the way, and you will have your own personal team who will be there for you anytime you need us.

How much carers are paid

  • shared Lives carers are paid £850 per week, per person
  • carers are self-employed and eligible for tax breaks and can claim mileage
  • access to our shopping rewards scheme
  • a discount at Wiltshire Council leisure facilities
  • membership of Shared Lives Plus
